Badge Replacement Guide

At ClearPoint, your security is our priority. If you lose your badge or key, here’s what to do to keep access secure and uninterrupted.
Step one: Report the loss immediately. Contact Security or your supervisor as soon as you realize your badge or key is missing.
Step 2: Fill Out the Replacement Form. Next, complete the Lost Badge or Key Replacement Form available on the company intranet or from the Security Office.
Submitting this form helps us deactivate your lost credentials and issue replacements promptly.
Step 3: Collect New Badge or Key. Once processed, pick up your new badge or key at the Security Office. Please note—there is a $10 replacement fee for lost badges or keys, so be sure to keep yours safe.
